Please fill in the form below and click submit if you have any team sheets or other paperwork you wish to include etc please scan them and forward them by email onto shane.supple.gda.cor@gaa.ie

(The Referee’s report should be completed accurately, completely and honestly)

You are requested to:
a) Be at the venue one hour before the start of the game
b) Arrange Umpires for the Match
c) Complete this form and return it (together with team sheets) within three days of the match
d) Any report that contains a disciplinary issue must be emailed to
shane.supple.gda.cor@gaa.ie no later than midday the following
day
e) In the event of being unable to act please notify (null) the person who assigned you this game immediately

Infractions (Team Officials)

Category Ia
*Abusive language towards a Referee, Umpire, linesman or sideline Official

Category IIa
Any type of physical interference with an opposing Player or team Official

Category IIIa
Minor physical interference with (e.g. laying a hand on, pushing pulling or jostling), threatening or abusive conduct towards or threatening language to, a referee, Umpire, linesman or sideline official.

Category IVa
Any type of assault on a Referee, Umpire, linesman or sideline Official

When reporting these infractions please provide details i.e. outline the words used by the person(s) who committed the infraction

Category I
Being ordered off on foot of a second Cautionable infraction (Football & Hurling)
Or a Cautionable infraction followed by a Cynical Behavior
Infraction (In Football}

Category II – Cynical Behavior infraction (Football Only)
Deliberately pulling down an opponent.
Deliberately tripping an opponent with the hand(s), arm, leg or foot.
Deliberately body colliding with an opponent after he has played the ball away or for the purpose of taking him out of a movement of play.
Threatening or using abusive or provocative language or gestures to an opponent or a team mate.
Remonstrating in an aggressive manner with a match Official

Category III
Striking or attempting to strike with arm, elbow, hand or knee.
Striking or attempting to strike with a Hurley, with minimal force.
Kicking or attempting to kick with minimal force.
Behaving in any way which is dangerous to an opponent. including deliberately pulling on or taking hold of a faceguard or any part of an opponent’s helmet (In Hurling)
Spitting at an opponent.
Contributing to a melee.
*Abusive language towards a Referee. Umpire, Linesman or
Sideline Official.

Category IV
Striking or attempting to strike with the head.
Striking with Hurley, either with force or causing injury.
Attempting to strike with Hurley, with force.
Kicking either with force or causing injury.
Attempting to kick, with force
Stamping.
Inflicting injury Recklessly by means other than those stated above.
Any type of assault on an Opposing Team Official.
An act by Deeds, Word or Gesture of Racist, Sectarian
or Anti-Inclusion/Diversity nature

Category V
Minor Physical interference (e.g. laying a hand on, pushing, pulling or jostling) with a Referee, Umpire, Linesman or Sideline Official.
*Threatening language to a Referee , Umpire, Linesman, or Sideline Official.
*Threatening or abusive conduct towards a Referee, Umpire, Linesman, or Sideline Official.

Striking or attempting to strike or any type of assault on, a Referee, Umpire, Linesman or Sideline Official,
